Our staff is comprised of Registered Nurses, Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, and Home Health Aides who are all committed to helping you or your loved one live a full life.

We provide services like medication management and support for chronic conditions. We also offer physical therapy for injuries or illnesses that may have resulted from an accident or surgery.

Nursing Care

 

When you need to be cared for in the comfort of your own home, our Registered Nurses (RNs) can help. They’ll travel to your home and provide skilled health care services. Our nurses use a variety of medical equipment to assess your needs and recommend treatment plans to get you feeling better fast. They also manage medications and lab work so you’re less likely to have drug interactions or adverse effects.

Physical Therapy Care

Affinity Bloom Home Heatlthcare’s Physical Therapists (PTs) provide a wide range of services to help you regain your independence and improve your mobility. Our Physical Therapists will work with you, your family and your doctor to develop an individualized treatment plan to help you achieve your goals.

Occupational Therapy Care


Our licensed Occupational Therapists work independently with their clients, tailoring treatment plans to meet their specific needs. They use a broad range of therapeutic services to help patients improve their abilities in activities of daily living (ADLs), such as home safety modifications, environmental adaptations, fall prevention, maintaining basic daily living skills (eating, meal preparation, medication reminders, grooming, dressing), restore functional mobility for safe transfers, improve basic motor functions and coordination, increase strength and dexterity for livings skills, instruct in the use of adaptive equipment, improve hand function, improve problem-solving skills, memory, and sequencing for independent living, and compensate for permanent loss of function.

Medicare pays 100% for intermittent skilled Occupational Therapy if the patient is homebound and the services are ordered by a physician.

Speech Therapy Care


Many adults have trouble speaking clearly, understanding what others say and using language to express their thoughts. After a stroke or other traumatic accident, adults may need speech therapy to improve their ability to speak and be understood.

Affinity Bloom Home Healthcare Speech Therapists will create a program to help you talk more easily and clearly by addressing the mechanics of speech production—articulation, pitch, fluency, and volume. We also address the nonverbal aspects of communication—how you say something as much as what you say. Our speech therapy programs are designed to restore or enhance speech capabilities.

Medical Social Worker


A Medical Social Worker (MSW) can help you resolve emotional or social problems at home. Our licensed MSWs will assess your situation and act on your needs and your family’s needs to access community resources and social services.

Medicare will pay 100% for intermittent skilled Medical Social Work if the patient is home bound, and the services are ordered by a physician.

Home Health Aide

 

Home Health Aides can help with daily living activities like bathing, dressing, grooming and meal preparation. They also remind you when to take your medicine. Affinity Bloom Home Healthcare’s Home Health Aides can come in for one hour a day to help you get stronger and more independent.

 

If you need more help, a Private Duty Home Health Aide can come into your home to provide care while you are recovering from an auto accident or a long-term illness.

 

Medicare pays for some of these services if they are ordered by your doctor.
